Late Open Reduction and Ligament Reconstruction of Old Unreduced Palmar Lunate

Abstract
The perilunate destabilization causes a marked disruption of the carpal anatomy and usually results in significant functional impairment. Despite the severity of this injury, diagnosis of acute perilunate dislocations are frequently unrecognized,
resulting in chronic form of this injury.
The treatment of old palmar lunate dislocations varies from leaving, alone, excision of dislocated lunate bone, proximal row carpectomy, primary arthrodesis to late open redution.
Except open reduction, all the above procedures do not produce a normal wrist and
complications are possible, especially in younger patient.
So we performed late open reduction and ligament reconstruction in 2 cases of old palmar lunate dislocations, 7 and 28 weeks after injury, respectively.
During open reduction, there no ligaments to be repaired. We reconstructed scapholunate ligament by four-bone ligament reconstruction using extensor carpi radialis brevis tendon and
lunotriquetral ligament using flexor carpi ulnaris tendon.
We reported the result of late open reduction and ligament reconstruction of 2 cases of old
palmar lunate dislocations.
